Job Overview

The Senior Manager, Care Coordination and Community Support will report directly to the Director of Care Management. This role is pivotal in managing the operations of the Enhanced Care Management (ECM) and Community Supports (CS) programs. You will play a crucial role in implementing new ECM target populations and Community Supports, contributing to a comprehensive, person-centered approach aimed at enhancing quality and care for members.

As a key decision-maker, you will ensure that objectives are met while adhering to budgetary, scheduling, and client satisfaction parameters. Your expertise in ECM and CS will be essential in overseeing regulatory evaluations and fostering vital relationships with stakeholders, including public health departments and partner organizations.

Key Responsibilities:

  • Assist in formulating and executing both short-term and long-term strategies for the implementation of ECM and CS benefits, focusing on holistic care for high-need members.
  • Coordinate projects by planning, sequencing tasks, establishing milestones, assigning responsibilities, facilitating workgroups, monitoring progress, and reporting on project status.
  • Lead the contracting, coordination, onboarding, and management of the ECM/CS network and associated programs.
  • Ensure operational compliance and reporting to meet and exceed state and federal standards.
  • Contribute to the planning and execution of new or enhanced benefits or pilot programs aimed at improving the implementation of CalAIM.
  • Provide direct leadership to ECM & CS staff, fostering a high-performance culture through coaching, skill development, and promoting a supportive team environment.
  • Manage operational matters and propose solutions to enhance services provided to members through collaboration with other departments, vendors, and provider networks.
  • Ensure that policies and procedures necessary for departmental operations comply with regulatory and accreditation standards and are reviewed annually.

Qualifications:

  • A Bachelor’s degree in policy, administration, or a healthcare-related field, or equivalent professional experience.
  • A minimum of 3 years of supervisory or management experience.
  • At least 3 years of experience collaborating with external stakeholders to achieve common objectives.
  • Experience in the delivery and implementation of large-scale health plan initiatives.
  • Understanding of the core functions of Medi-Cal and CalAIM.
  • Familiarity with best practices in case management and care coordination within health plan and integrated delivery system models.
