Position Overview

Community Options, Inc. is a prominent national non-profit organization dedicated to providing essential services to individuals with disabilities across multiple states.

We are currently in search of Full-Time and Part-Time Day Services Coordinators. This position is focused on delivering personalized support aimed at assisting participants in navigating and overcoming the challenges faced by individuals with disabilities within their communities.

Compensation: Starting pay is set at $19.00 per hour.

We are pleased to offer a $1,000 sign-on bonus for full-time staff and a $500 sign-on bonus for part-time staff (for those working 16 or more hours per week), which becomes available after 90 days of satisfactory employment. Additionally, a $700 employee referral bonus for full-time employees and a $350 referral bonus for part-time employees is also offered, contingent upon meeting the 90-day satisfactory work and attendance criteria.

Key Responsibilities:
  • Encourage and assist individuals in enhancing their independence.
  • Utilize personal transportation for work-related duties.
  • Engage in one-on-one interactions with clients as part of our team.
  • Regularly meet with waiver participants in various settings including their homes, community locations, or office environments.
  • Collaborate with Case Managers to design and implement programs that address the social, emotional, physical, and personal needs of each individual.
  • Provide continuous support for community integration initiatives.
  • Document progress towards achieving community integration goals.
  • Help establish a supportive network between individuals and their communities.
  • Ensure completion of medical consultation forms by physicians for each medical appointment attended.
  • Administer medications as required and maintain necessary documentation.
  • Coordinate leisure and volunteer opportunities for participants.

Qualifications:
  • High School Diploma or GED is required.
  • A valid Driver's License is necessary.
