Role of a Nurse Case Manager

A Nurse Case Manager plays a crucial role as an advocate for our patients, acting as a bridge between clients, their families, and healthcare providers. Responsibilities include assessing, planning, implementing, coordinating, and evaluating patient care needs. You will review and interpret medical records, diagnoses, treatment options, and findings to ensure comprehensive care. Additionally, you will facilitate access to resources and benefits available through government programs, community services, and educational institutions. Building strong relationships with patients, their families, and community providers is essential for success in this role.

Qualifications We Seek

PLEASE NOTE: THIS POSITION REQUIRES IN-OFFICE PRESENCE AT OUR MISSION VALLEY LOCATION; REMOTE WORK IS NOT AVAILABLE. CANDIDATES MUST RESIDE IN SAN DIEGO COUNTY OR BE PLANNING A MOVE TO SAN DIEGO WITHOUT RELOCATION ASSISTANCE.

Applicants must possess a valid California Nursing License and a degree in Nursing, preferably a Bachelor's degree, although an Associate's degree will be considered. A minimum of two years of acute nursing experience is required, with a preference for candidates with rehabilitation experience. The ability to travel to various appointments across San Diego is necessary. We value self-motivated individuals with excellent communication skills, both written and verbal. Teamwork is essential at RCC; candidates should be capable of managing multiple case management patients independently and collaboratively. Fluency in additional languages is a plus.
