Responsibilities of a Nurse Case Manager
A Nurse Case Manager plays a crucial role in advocating for our patients' well-being, acting as a bridge between clients, their families, and healthcare providers. Key responsibilities include:
- Assessing, planning, implementing, coordinating, and evaluating patient care needs.
- Reviewing and interpreting medical records, diagnoses, treatment options, and findings.
- Coordinating resources and benefits available to patients through various channels, including government and community services.
- Building strong relationships with patients, families, and community resources.
Qualifications We Seek
This position is in-office, and candidates must reside in the local area. We require:
- A valid California Nursing License and a degree in Nursing (Bachelor's preferred, Associate's acceptable).
- A minimum of two years of acute nursing experience; rehabilitation experience is a plus.
- The ability to travel to various appointments across the region.
- Strong communication skills, both written and verbal, with a self-motivated approach.
- Fluency in additional languages is advantageous.
