About the Role

We are seeking a highly experienced and skilled Senior Manager, Enhanced Care Management and Community Supports to join our team at San Francisco Health Plan. This is a critical leadership position that will oversee the operation of our Enhanced Care Management (ECM) and Community Supports (CS) benefits.

Key Responsibilities
  • Strategic Leadership: Develop and execute short-term and long-term strategies for the implementation of ECM and CS benefits, ensuring alignment with our organization's goals and objectives.
  • Project Coordination: Coordinate and manage projects related to ECM and CS, including planning, sequencing, setting milestones, and reporting project status.
  • Network Management: Drive contracting, coordination, onboarding, and management of ECM/CS network and programs.
  • Operational Compliance: Ensure operational compliance and reporting to satisfy and exceed state and federal standards.
  • Staff Development: Provide guidance and coaching to ECM & CS staff, promoting a high-performing team and creating opportunities for staff growth and development.
  • Policies and Procedures: Ensure that related policies and procedures necessary to operate the department comply with regulatory and accreditation requirements.
Requirements
  • Education: Bachelor's degree in policy, administration, or health care related field, or equivalent work experience.
  • Experience: 3 or more years of supervisory or management experience, with a proven track record of success in leading teams and driving results.
  • Skills: Strong leadership and project management skills, with experience in collaborating with external stakeholders and driving large-scale health plan initiatives.
  • Knowledge: Knowledge of Health Plan Core functions of Medi-Cal and CalAIM, as well as Health Plan/Integrated Delivery system models of case management and care coordination best practices.

  • Competitive Salary: $126,000 to $142,000 per year.
