Position Overview:
The Senior Manager of Care Coordination and Community Services will report directly to the Director of Care Management. This role is essential in overseeing the operations of the CalAIM Enhanced Care Management (ECM) and Community Supports (CS) benefits.

Key Responsibilities:
- Facilitate the implementation of new ECM target populations and Community Supports, contributing to a comprehensive, person-centered reform aimed at enhancing care quality for members.
- Ensure that strategic objectives are met while adhering to budgetary constraints, timelines, and stakeholder expectations.
- Act as a subject matter expert for ECM and CS, managing regulatory audits and fostering critical relationships with key stakeholders, including public health departments and contracted providers.

Work Environment:
This position is primarily based in an office setting, with a requirement to be onsite a minimum of four days per month.

Compensation:
Salary range is $126,000 to $142,000.

Primary Duties:
- Develop and execute short-term strategies for the ECM and CS benefits while contributing to long-term planning for integrated care for high-need members.
- Oversee project coordination, including planning, setting milestones, assigning tasks, facilitating workgroups, and monitoring progress.
- Drive the contracting and management of the ECM/CS network and programs, ensuring operational compliance and reporting that meets or exceeds state and federal standards.
- Guide ECM & CS staff to foster a high-performing team, encouraging individual goal setting, skill development, and a positive team culture.
- Address operational matters and propose solutions to enhance services provided to members.

Qualifications:
- A Bachelor's degree in policy, administration, or a healthcare-related field, or equivalent experience.
- At least three years of supervisory or management experience.
- Proven experience in collaborating with external stakeholders to achieve common objectives.
- Familiarity with large-scale health plan initiatives and knowledge of Medi-Cal and CalAIM core functions.
