Position Overview

As a Social Work Case Manager, you will be integral in delivering holistic social services and assistance to individuals and families facing challenges. This role demands a profound dedication to our clients, characterized by compassion and a proactive mindset in addressing issues. You will collaborate closely with clients and their families to evaluate their requirements, formulate care strategies, coordinate available resources, and advocate for their welfare. Supporting children with developmental disabilities represents a critical mission, emphasizing the importance of your commitment to ensuring that our clients' needs are met effectively.

Key Responsibilities:
  • Handle initial intake inquiries and connect families or caregivers with suitable referrals.
  • Conduct comprehensive evaluations of clients' social, emotional, financial, and environmental circumstances.
  • Provide crisis intervention and emotional support to clients and their families as required.
  • Develop personalized care strategies in partnership with clients, families, and multidisciplinary teams, establishing objectives and interventions tailored to their unique needs.
  • Communicate client progress by facilitating and/or participating in interdisciplinary meetings, updating the treatment team on case status, and identifying obstacles to successful treatment.
  • Serve as a liaison among the client's interdisciplinary treatment team, guardians, and external service providers to address care needs.
  • Collaborate effectively with interdisciplinary teams, including healthcare professionals, social workers, psychologists, and community organizations.
  • Assist in the admission identification process by reviewing and reporting on potential inpatients.
  • Coordinate and connect clients with relevant support services available at the center and statewide, such as outpatient therapies, developmental disability services, childcare programs, and other local resources.
  • Advocate for clients' rights and access to services across various systems, including healthcare, education, and social services.
  • Monitor clients' progress towards established goals, regularly reassessing needs and modifying care plans as necessary.
  • Work closely with the Oklahoma Department of Human Services, including Child Welfare and Developmental Disability Services, in caring for children in state custody and facilitating transition planning for placement opportunities.
  • Maintain precise and confidential client records, documentation, and reports according to hospital and departmental policies.
  • Prepare for client discharges by reviewing and adjusting discharge plans, coordinating requirements, outlining achieved goals, and providing necessary resources.
  • Assess training needs for families, caregivers, schools, or other service providers regarding understanding the client's treatment plan and coordinating applicable training with internal and external entities.
  • Participate in various agency, community, and state committees aimed at achieving departmental and agency objectives.
  • Assist in maintaining and evaluating quality assurance measures for the department.
  • Perform additional duties as assigned.
Required Qualifications:
  • Bachelor's degree in Social Work (BSW) from an accredited institution.
  • Demonstrated experience in case management, social work, or a related field is preferred.
  • Strong understanding of social services, community resources, and public assistance programs.
  • Excellent interpersonal skills, with the ability to establish trust and rapport with diverse clients.
  • Effective verbal and written communication skills.
  • Ability to work independently, prioritize tasks, and manage time efficiently.
  • Knowledge of cultural competence and sensitivity to diverse populations.
  • Commitment to ethical practices and maintaining client confidentiality.
Preferred Qualifications:
  • Master's degree in Social Work (MSW) from an accredited program.
  • Experience working with individuals with developmental disabilities.
