Job Overview

POSITION SUMMARY

The Care Manager-Liaison plays a crucial role in providing outreach support, connection, and advocacy for individuals within a designated correctional environment. This position involves delivering Case Management services to adults, youth, and their families within the community and associated programs. Care Managers regularly collaborate with various team members, including healthcare professionals and treatment providers, to evaluate and assist individuals facing mental health, emotional, or substance use challenges, including issues related to alcohol, tobacco, and other drugs.

KEY RESPONSIBILITIES

  • Act as a connector for clients by coordinating their services within the organization and addressing their diverse Case Management needs in the community.
  • Monitor clients' progress in various programs, such as mental health initiatives, and adjust their Treatment Plans as necessary.
  • Serve as a liaison between clients and external state and city personnel, advocating for clients and ensuring adherence to program standards.
  • Facilitate approved services, ensuring effective communication among service providers and advocating for clients to ensure treatment and reintegration plans are implemented.
  • Collaborate directly with inpatient hospital staff to develop tailored discharge and aftercare plans for clients.
  • Connect clients receiving mental health treatment with community resources, including financial assistance, housing, health services, and daily living skills.
  • Coordinate with other internal programs regarding client care, including medical clinics and therapeutic services.
  • Prepare, plan, and implement direct care through individual and group rehabilitative services, monitoring client progress.
  • Provide crisis intervention to ensure client safety, consulting with relevant stakeholders as needed.
  • Complete service plans and necessary paperwork promptly, ensuring compliance with organizational standards.
  • Participate in treatment team meetings, contributing valuable insights for client care.
  • Maintain competencies related to co-occurring disorders through ongoing education and practical application.
  • Engage in staff meetings and training sessions to stay informed about agency policies and procedures.
  • Assist in developing and executing program goals and objectives.
  • Uphold confidentiality and inform clients of their rights, securing necessary authorizations for information sharing.
  • Establish strong working relationships with local, state, and federal agencies.
  • Promote community education initiatives to raise awareness about mental health issues.
  • Support the facilitation of Care Plans for clients within the health team.
  • Perform additional duties as assigned.

QUALIFICATIONS

  • Minimum of two years' experience in community services and health agencies, particularly with low-income and diverse populations.
  • Current certification as a Behavioral Health Case Manager II (BHCM II) is preferred.

OR

  • Bachelor's degree in a relevant mental health field.
  • Bilingual candidates are preferred.
  • Ability to exercise independent judgment, demonstrate patience, and maintain effective relationships with clients and colleagues.
  • Basic computer skills and a valid driver's license with a clean record are required.
