Job Overview

POSITION SUMMARY

The Care Manager-Liaison plays a crucial role in providing outreach support, connection, and advocacy for individuals within the designated correctional facility. This position is responsible for delivering Case Management services to adults, youth, and their families, both in the community and through Red Rock BHS programs. Care Managers frequently collaborate with various staff members, including the Health Team and treatment providers, to evaluate and address the needs of individuals facing mental health, emotional, or substance use challenges, including issues related to alcohol, tobacco, and other drugs.

KEY RESPONSIBILITIES

  • Act as a primary link for consumers by coordinating their services within Red Rock BHS and addressing their diverse Case Management requirements in the community.
  • Track individual progress in various programs, such as the Mental Health Court Program, and adjust Treatment Plans as necessary.
  • Serve as an intermediary between Red Rock consumers and external state and city personnel, advocating for consumers and ensuring adherence to program standards.
  • Facilitate communication among service providers, DHS, and other referral sources; advocate for consumers to ensure compliance with treatment and reintegration plans, and assist in arranging follow-up support services.
  • Collaborate directly with inpatient hospital staff to create tailored discharge and aftercare plans for consumers.
  • Connect consumers receiving mental health treatment at Red Rock with additional community resources, including financial, housing, health, and daily living support; provide referral linkage and advocacy as needed.
  • Coordinate with other Red Rock programs regarding consumer care, including Med Clinic, therapy, and diagnostic services.
  • Plan, facilitate, and implement direct care services for consumers through individual and group rehabilitative treatment and supportive counseling.
  • Provide crisis intervention to ensure consumer safety by consulting with consumers, staff psychiatrists, family members, and emergency units; complete necessary documentation for hospitalization.
  • Ensure timely completion of service plans and prior authorizations, along with all required paperwork.
  • Participate in treatment team meetings, contributing relevant information for consumer care.
  • Maintain core competencies in managing co-occurring disorders through ongoing education and skill application in treatment.
  • Engage in staff meetings, workshops, and seminars to stay informed about agency policies, rules, and procedures; participate in continuous in-service training.
  • Assist in developing and implementing programmatic goals and objectives.
  • Uphold confidentiality standards and inform consumers of their rights, obtaining necessary authorizations for information sharing.
  • Establish and nurture working relationships with local, state, and federal agencies.
  • Promote community education initiatives to enhance awareness of Post-Traumatic Stress Disorder (PTSD) and Acute Mental Health Crises.
  • Support the facilitation of Care Plans for consumers within the Red Rock Health Team.
  • Perform additional duties as assigned.

QUALIFICATIONS

  • Minimum of two years' experience in community services and health agencies, particularly with low-income and minority populations, demonstrating an understanding of cultural diversity.
  • Current certification as a Behavioral Health Case Manager II (BHCM II) is preferred.

OR

  • Bachelor's degree in a relevant mental health field.
  • Bilingual candidates are preferred.
  • Ability to exercise independent judgment, tact, patience, and understanding while maintaining effective relationships with consumers, staff, and community organizations.
  • Completion of all in-service and external training requirements is necessary.
  • Valid driver's license with a clean driving record and current auto insurance is required.
