Position Overview

The J.D. McCarty Center is a dynamic organization dedicated to enhancing treatment methodologies for our patients and their families. We cater to children aged 0-21 across various environments, including inpatient, outpatient, and educational settings.

The C.A.R.E. Program at the J.D. McCarty Center is in search of a Case Manager to collaborate with patients and their families to identify their requirements and aspirations for ongoing care and development. This role involves linking patients and/or their caregivers with suitable support services and resources available at the J.D. McCarty Center, within the community, and statewide.

Compensation: Starting salary of $42,500 annually, in addition to paid time off, insurance, and retirement benefits.

Work Schedule: No on-call or weekend hours required.

This position entails some travel responsibilities, necessitating a valid Oklahoma driver's license. A bachelor's degree in a human services discipline is required, with preference given to candidates holding a BSW or MSW. Proficiency in Microsoft Office Suite, Office 365, and Zoom is essential. This role focuses on caregiver support and does not involve direct patient care.

Key Responsibilities:

  • Facilitates connections between patients and their families with relevant support services and resources available at the J.D. McCarty Center and statewide. Provides guidance on available supports, informs about application processes, and assists families in accessing services.
  • Evaluates training and development needs for families, caregivers, schools, or other service providers concerning understanding intellectual and developmental disabilities (ID/DD) and behavioral support, referring them to the CARE Team Clinician for further assistance.
  • Maintains comprehensive documentation within specified timeframes and agency guidelines, including progress notes, staffing reports, admission and discharge summaries, and billing statements.
  • Coordinates outreach services with CARE team members and other Center staff as necessary for training and support services in various settings, including home and school. Attends outreach appointments and facilitates collaboration between treatment teams, external resources, and families.
  • Participates in various agency, community, and state committees aimed at achieving departmental and organizational goals.
  • Enhances patient care by assessing treatment needs, developing, monitoring, and evaluating treatment plans, and facilitating interdisciplinary approaches and interagency collaborations.
  • Supports caregiver and patient objectives by conducting intake interviews, determining the necessity for therapeutic medical, psychosocial, and psychiatric evaluations, and maintaining treatment goals and plans.
  • Establishes treatment plans by setting expectations with families, coordinating services, arranging resources, assisting with service applications, and completing referrals for services at JDMC and within the community.
  • Monitors case status by verifying participation, observing treatment responses, advocating for necessary services, obtaining additional resources, intervening in crises, and assisting in developing crisis plans.
  • Communicates client progress by attending interdisciplinary meetings, updating the treatment team on case status, and identifying barriers to successful treatment.
  • Prepares for client discharges by reviewing and adjusting discharge plans as necessary, coordinating discharge requirements, outlining achieved goals, and providing resources.
  • Handles intake calls and connects families with appropriate referrals. Monitors new CARE referrals and follows up with guardians or Child Welfare Workers, ensuring proper documentation of new referrals.
  • Assists in maintaining and evaluating quality assurance measures.
  • Completes various weekly and monthly reports for the CARE team.
  • Processes internal referrals to JDMC services, such as outpatient therapies and clinics.
  • Performs additional duties as assigned.

Preferred Qualifications:

Education and experience requirements include a bachelor's degree in a human services field and one year of experience working directly with individuals with intellectual and/or developmental disabilities. Alternatively, possession of a valid permanent Oklahoma nursing license and one year of relevant experience is acceptable. A bachelor's degree in Social Work is preferred. Responsibilities may require work-related travel, and preference will be given to bilingual candidates. Proficiency in Microsoft Office Suite and O365 is necessary.
