Job Overview

POSITION SUMMARY

The role involves providing outreach assistance, connection, and advocacy for individuals at designated correctional facilities. This position encompasses Case Management services for adults, youth, and their families within the community and Red Rock BHS programs. Care Managers engage frequently with other personnel, including the Health Team and treatment providers, to evaluate and address the needs of individuals facing mental, emotional, or substance use challenges, including issues related to alcohol, tobacco, and other drugs.

KEY RESPONSIBILITIES

  • Act as a connector for clients by coordinating their services within Red Rock BHS and addressing their diverse Case Management requirements in the community.
  • Track clients' progress in various programs, such as the Mental Health Court Program, and adjust their Treatment Plans as necessary.
  • Serve as a liaison between Red Rock clients and various external state and city personnel, advocating for clients and ensuring adherence to Program standards.
  • Coordinate authorized services, facilitating communication among various service providers, and advocate for each client to ensure compliance with treatment and reintegration plans.
  • Establish direct communication with inpatient hospital staff to develop specific discharge and aftercare plans for clients.
  • Link clients receiving mental health treatment at Red Rock with community resources, including financial, housing, health, and daily living skills; provide referral linkage and advocacy.
  • Collaborate with other Red Rock programs concerning client care, including Med Clinic, therapy, and diagnostic services.
  • Prepare, plan, and implement direct care for clients through group and individual rehabilitative services; arrange appropriate group placements and monitor progress.
  • Provide crisis intervention to ensure client safety by consulting with clients, staff psychiatrists, family members, and emergency units; manage necessary hospitalization paperwork.
  • Complete all service plans and prior authorizations promptly; manage all required documentation efficiently.
  • Participate in treatment team meetings, contributing relevant information for client care.
  • Maintain expertise in working with co-occurring disorders through ongoing education and practical application of skills.
  • Attend staff meetings, workshops, and seminars to stay informed about agency policies, rules, and procedures; engage in continuous in-service training.
  • Assist in the formulation and execution of programmatic goals and objectives.
  • Uphold confidentiality and inform clients of their rights while obtaining necessary authorizations for information exchange.
  • Foster collaborative relationships with local, state, and federal agencies.
  • Promote community education initiatives to enhance awareness of Post-Traumatic Stress Disorder (PTSD) and Acute Mental Health Crises.
  • Support the facilitation of Care Plans for clients within the Red Rock Health Team.
  • Perform additional duties as assigned.

QUALIFICATIONS

  • Minimum of two years' experience in community services and health agencies, particularly with low-income and minority populations, demonstrating an understanding of cultural diversity.
  • Current certification as a Behavioral Health Case Manager II (BHCM II) is preferred.

OR

  • Bachelor's degree in an approved mental health-related field.
  • Bilingual candidates are preferred.
  • Ability to exercise independent judgment, tact, patience, and understanding; maintain effective working relationships with clients, staff, and community organizations.
  • Basic computer skills are required.
  • Valid Oklahoma Driver License with a clean Motor Vehicle Record and current Auto Insurance.
