Child Welfare Case Coordinator

As a Family Services Case Manager, you will be essential in delivering extensive social services and assistance to individuals and families requiring support.
Your responsibilities will include collaborating closely with clients and their families to evaluate their needs, formulate care strategies, coordinate available resources, and advocate for their welfare.
Children with developmental disabilities represent one of the most at-risk groups in our community, making it crucial to possess a strong commitment and passion for ensuring their needs are met as part of the mission of the State of OK JD McCarty Center.
Compensation:
$50,000 starting salary, adjusted based on education and experience.
Key Responsibilities:
- Manage intake inquiries and connect families or caregivers with suitable referrals.
- Conduct comprehensive evaluations of clients' social, emotional, financial, and environmental requirements.
- Provide crisis intervention and emotional support to clients and their families as necessary.
- Create personalized care plans in partnership with clients, families, and multidisciplinary treatment teams, establishing objectives and interventions tailored to their unique situations.
- Report on client progress by facilitating and/or participating in interdisciplinary meetings, updating the treatment team on case status, and identifying obstacles to successful treatment.
- Serve as a liaison among the client's interdisciplinary treatment team, guardians, and external service providers for care needs.- Collaborate effectively with interdisciplinary teams, including healthcare professionals, social workers, psychologists, and community organizations.
- Assist in the admission identification process by reviewing and reporting on potential inpatients. Help with scheduling and coordinating inpatient admissions as needed.
- Connect clients with appropriate support services available at the State of OK JD McCarty Center and throughout the region, such as outpatient therapies, Developmental Disability Services, childcare programs, Supplemental Security Income (SSI), and other local and statewide resources.
- Advocate for clients' rights and access to services across various systems, including healthcare, education, and social services.
- Monitor clients' progress towards their goals, regularly reassessing needs and modifying care plans as required.- Collaborate closely with the Oklahoma Department of Human Services (including Child Welfare, Child Protective Services, and Developmental Disability Services) in caring for children in state custody and facilitating transition planning for placement opportunities.
- Maintain precise and confidential client records, documentation, and reports in accordance with hospital and departmental policies.
- Prepare discharge plans by reviewing and modifying them as necessary, coordinating discharge requirements, outlining achieved goals, and providing resources.
Facilitate transition planning for clients upon discharge.- Assess training needs for families, caregivers, schools, or other service providers regarding understanding the client's treatment plan and coordinate any relevant training with internal and external entities.
- Participate in various agency, community, and state committees aimed at achieving departmental and agency objectives, mission, and vision.
- May assist in maintaining and evaluating quality assurance measures for the department.- Perform other duties as assigned.
Required Qualifications:
- Bachelor's degree in Social Work (BSW) from an accredited institution.
- Proven experience in case management, social work, or a related field is preferred.
- Strong understanding of social services, community resources, and public assistance programs.
- Excellent interpersonal skills with the ability to establish trust and rapport with diverse clients.
- Effective communication skills, both verbal and written.
- Ability to work independently, prioritize tasks, and manage time efficiently.
- Knowledge of cultural competence and sensitivity to diverse populations.
- Commitment to ethical practices and maintaining client confidentiality.
Preferred Qualifications:
- Master's degree in Social Work (MSW) from an accredited program.
- Knowledge and experience working with individuals with developmental disabilities.

At the State of OK JD McCarty Center, patients undergo a 30-day evaluation period during which they are assigned a treatment team that includes nursing, physical therapy, occupational therapy, speech therapy, nutritional services, social services, psychological services, behavioral analysis, and recreational therapy.
Our expansive campus features an on-site elementary, intermediate, and high school staffed by qualified educators.
Patients may continue their treatment beyond the initial 30 days as inpatients based on their progress or placement needs.
