A DAY IN THE LIFE OF AN RN CASE MANAGER
As an RN Case Manager, your primary responsibility is to ensure that our patients receive top-notch nursing care. You will conduct initial assessments to determine the need for hospice services within 48 hours of their election and collaborate with the attending physician and care team to complete a thorough evaluation. After assessments are finalized, you will coordinate and implement the care plan, continuously monitoring patients and adjusting their care strategies based on evolving needs, including physical, psychological, social, and spiritual factors.
To maintain seamless patient care, you will engage in regular communication with the attending physician and interdisciplinary care team members. You will also keep accurate patient records and fulfill documentation requirements to comply with regulatory standards. Providing essential leadership, you will guide caregivers and volunteers while supervising LPNs and hospice aides.
Prioritizing patient safety and well-being, you will adhere to infection control protocols and report any observed abuse. You will also provide direct care as necessary and assist with discharge processes. Opportunities for professional development through agency and community programs, involvement in special projects, and participation in training activities will be available, allowing you to make a significant impact on the lives of your patients.
QUALIFICATIONS FOR AN RN CASE MANAGER
- Valid Oklahoma Registered Nurse license
- Minimum of one year of clinical experience
- Reliable transportation and vehicle liability insurance
While experience in hospice, home health, critical care, or emergency services is preferred, it is not mandatory. We seek individuals who can adapt to changing circumstances, enjoy mentoring others, and demonstrate respect and patience for those who are ill, disabled, elderly, or emotionally distressed. Strong interpersonal and communication skills, creativity, critical thinking, integrity, and initiative are essential traits for this nursing role.
WORK SCHEDULE
This full-time nursing position typically operates on a Monday to Friday, 8 am to 5 pm schedule.
