Position Overview:

Key Responsibilities:

As a Program Coordinator, you will:

  • Ensure quality, health, safety, and client-centered approaches for individuals receiving community living services.
  • Supervise, support, and train staff in their roles assisting individuals served.
  • Maintain compliance with federal, state, and local regulations while promoting opportunities for individuals to engage in community life.

Essential Duties:

  • Develop and implement Individual Service Plans (ISPs) and assist in creating the Plan of Care for each individual served.
  • Ensure that the Plan of Care addresses all client needs and communicates effectively with appropriate staff.
  • Oversee assessments, planning, implementation, and review of person-centered support plans.
  • Coordinate the delivery of services provided by Benchmark and other service providers.
  • Support individuals in expanding their participation in community activities.
  • Provide on-call direct support and emergency coverage as needed.
  • Participate in the recruitment, training, and supervision of Team Leaders, Supported Living Managers, and Direct Support Professionals.
  • Schedule staff according to the Plan of Care requirements.
  • Ensure accurate and timely completion of ISPs, client financials, and documentation.
  • Report any incidents within required time frames.
  • Act as a liaison with external case managers, families, and other agencies.
  • Adhere to all standards to ensure the health and safety of individuals served.
  • Report to work as scheduled.

Qualifications:

  • Bachelor's degree in human services with at least one (1) year of experience working with individuals with developmental disabilities, or five (5) years of relevant experience.
  • Management experience is preferred.
  • Valid driver's license is required.
