Position Overview:

Benchmark Human Services, established in 1960 and based in Fort Wayne, Indiana, has emerged as a leading organization in the field of intellectual and developmental disabilities (IDD) and behavioral health. Key Responsibilities:

As a Program Coordinator, you will be responsible for ensuring quality, health, safety, and client-centered approaches for individuals in community living services. Your duties will include:

  • Overseeing, supporting, and training staff to assist individuals served.
  • Ensuring compliance with federal, state, and local regulations while promoting opportunities for community participation.
  • Developing and implementing Individual Service Plans (ISPs) and assisting in the creation of Plans of Care.
  • Coordinating the delivery of services provided by Benchmark and other service providers.
  • Facilitating opportunities for individuals to engage in community life.
  • Providing on-call direct support and emergency coverage as needed.
  • Participating in the recruitment, training, and supervision of Team Leaders, Supported Living Managers, and Direct Support Professionals.
  • Scheduling staff in alignment with the Plan of Care.
  • Ensuring accurate and timely completion of documentation, including ISPs, client financials, and compliance checks.
  • Reporting incidents within required timeframes and serving as a liaison with external case managers, families, and other agencies.
  • Adhering to all health and safety standards to protect the well-being of individuals served.
  • Maintaining a reliable work schedule.

Qualifications:

  • A Bachelor's degree in human services or a related field, with at least one (1) year of experience working with individuals with developmental disabilities, or five (5) years of relevant experience.
  • Management experience is preferred.
  • A valid driver's license is required.
