Key Responsibilities:

The Supported Living Manager (SLM) plays a crucial role in overseeing the daily operations of Supported Living homes. This position involves supervising staff and ensuring a fulfilling daily experience for our clients.

Essential Duties:

  • Supervise, train, and mentor staff in compliance with state and federal regulations, as well as internal policies and procedures.
  • Collaborate with a multidisciplinary team to develop each client's Individual Support Plan (ISP) and Person-Centered Plan (PCP).
  • Engage in direct service provision for clients as needed.
  • Maintain accurate documentation as required by Benchmark and regulatory standards, ensuring proper client file management.
  • Oversee client budgets and ensure timely payment of bills, assisting clients in maintaining Medicaid eligibility.
  • Communicate effectively with families, case managers, healthcare professionals, and other stakeholders.
  • Participate in team meetings, training sessions, and various client-related meetings as assigned.
  • Implement training programs tailored to each client's ISP.
  • Adhere to behavior support policies and implement approved behavior support strategies.
  • Involve in the hiring and disciplinary processes for assigned staff.
  • Ensure proper administration of medications and assist clients with their healthcare needs.
  • Coordinate and accompany clients to medical appointments, ensuring follow-up care is completed.
  • Plan and facilitate community-based recreational activities suitable for clients.
  • Conduct training for new and existing staff as per required protocols.
  • Respond promptly to on-call situations as they arise.
  • Report any suspected abuse, neglect, or exploitation to the appropriate authorities.
  • Maintain necessary documentation for employment, including certifications and training as mandated by federal and state regulations.
  • Be physically capable of lifting up to 50 pounds and standing or walking for extended periods.

Qualifications:

  • Bachelor's degree is preferred.
  • Minimum of two years of experience working with individuals with developmental disabilities.
  • At least one year of supervisory experience is preferred.
  • Current CPR and First Aid certifications.
  • Valid driver's license and auto insurance required.
  • Completion of necessary training in areas such as interviewing, hiring, and employee relations.
