Key Responsibilities
  • Assist in planning, organizing, and implementing social service programs to meet the needs of our residents.
  • Conduct interviews with residents and families to gather social history and develop personalized care plans.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support for residents.
  • Provide consultation and coaching to staff members to enhance their understanding of social service needs.
  • Develop and implement written plans of care for each resident, identifying problems, needs, and goals.
  • Ensure accurate and timely documentation of resident progress and services provided.
  • Recruit, interview, and select personnel for the community or department as needed.
  • Participate in community surveys and develop plans of correction for department deficiencies.
Requirements
  • Must possess a strong understanding of social service principles and practices.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and staff.
  • Ability to work independently and make independent decisions.
  • Flexibility and adaptability in a fast-paced environment.
  • Must be able to see, hear, and smell, or use prosthetics to enable these senses to function adequately.
  • Ability to lift, push, pull, move, and carry a minimum of 25 pounds.
  • Subject to exposure to noise, infectious waste, diseases, and conditions, including TB, HIV, and HEP B viruses.
