Position Overview:

We are currently seeking a Supported Living Manager for our operations in South Bend, IN.

KEY RESPONSIBILITIES:

The Supported Living Manager (SLM) plays a crucial role in overseeing and managing the operations of Supported Living homes. This includes supervising staff and ensuring a fulfilling daily experience for clients.

ESSENTIAL DUTIES:

  • Supervise, train, and mentor staff in compliance with state and federal regulations, as well as Benchmark Human Services policies and procedures.
  • Collaborate with a multidisciplinary team to develop each client's Individual Support Plan (ISP) and Person-Centered Plan (PCP).
  • Engage in direct service delivery for clients as needed.
  • Maintain accurate documentation as required by Benchmark and regulatory standards, ensuring client files are up to date.
  • Manage client budgets and ensure timely payment of bills, assisting clients in maintaining Medicaid eligibility.
  • Communicate effectively with families, case managers, healthcare professionals, and other stakeholders.
  • Participate in team meetings, ISP meetings, and other relevant gatherings.
  • Implement training programs as outlined in each client's ISP.
  • Adhere to behavior support policies and implement approved behavior support strategies.
  • Involve in the hiring and disciplinary processes for assigned staff.
  • Ensure proper administration of medications and assist in addressing clients' healthcare needs.
  • Coordinate and accompany clients to medical appointments and follow up as necessary.
  • Plan and facilitate community-based recreational activities suitable for clients.
  • Conduct training for new and existing staff as required.
  • Respond promptly to on-call situations as they arise.
  • Report any suspected abuse, neglect, or exploitation to the appropriate supervisor.
  • Maintain necessary documentation for employment, including certifications and training compliance.
  • Be capable of lifting up to 50 pounds and standing or walking for extended periods.

QUALIFICATIONS:

  • Bachelor's degree is preferred.
  • Minimum of two years of experience working with individuals with intellectual disabilities.
  • At least one year of supervisory experience is preferred.
  • Current CPR and First Aid certifications.
  • Valid driver's license and auto insurance required.
  • Completion of necessary training in areas such as interviewing, hiring, and employee relations.
