POSITION SUMMARY
The Supportive Housing Case Coordinator plays a crucial role in delivering essential services to residents. These services encompass individual and group counseling, advocacy, referrals, and follow-up support, as well as assistance with home management and financial planning. A deep understanding of the challenges faced by individuals experiencing homelessness is vital.

Moreover, the Case Coordinator will establish necessary connections with community organizations to ensure residents receive appropriate placements in permanent housing, access to aftercare services before transitioning from the agency, and support in obtaining vocational and educational opportunities, as well as public assistance and Medicaid benefits.

COMPENSATION:
$42,000 - $47,000

KEY RESPONSIBILITIES
Manage a caseload of 25-30 families.

Conduct intake assessments to evaluate residents for admission using the DHS CARES system and as directed by the organization.


Engage residents in both initial and ongoing treatment through individual and group counseling, under the clinical supervision of the Social Work Supervisor.

Regularly develop independent living plans in the DHS CARES system.

Monitor case plans to ensure they meet individual client needs and enhance their ability to function independently.


Build and maintain housing referral resources, assisting residents in locating and securing permanent housing in collaboration with the Housing Specialist.

Support residents in accessing community-based resources, including aftercare services.

Evaluate residents' vocational and educational needs, interests, and aptitudes through interviews and assessments. Establish and maintain placement and follow-up processes as necessary.

Foster relationships with DHS, HRA, Public Assistance, and Medicaid personnel to assist eligible residents in securing benefits by acting as an advocate through phone or in-person communication.

Assist clients in developing savings plans as applicable.

Conduct weekly inspections of all units, including those involving families with children from birth to six months of age, on a biweekly basis.

Explore family reunification options when transitioning out of shelter.

Continuously assess the safety and well-being of clients.

Schedule appointments for residents' medical examinations.


Collaborate with residents to create a contract outlining mutually agreed-upon goals and objectives to be achieved within a specified timeframe.

Facilitate psychoeducational groups alongside direct service staff.

Assist in coordinating both on-site and off-site services.

Participate in training sessions aimed at enhancing clinical skills relevant to the client population.

Attend weekly individual and group supervision sessions, monthly case conferences, and staff meetings.

Function independently during facility coverage, including taking appropriate action in crisis situations according to agency protocols.

Perform additional duties as assigned by the Social Work Supervisor.

EDUCATION AND CERTIFICATIONS
Bachelor's Degree in Human Services.

ESSENTIAL SKILLS AND EXPERIENCE
Bilingual candidates are strongly encouraged to apply.

Minimum of two (2) years of counseling experience, preferably in a residential shelter setting.

At least three (3) years of crisis counseling experience in a residential shelter for homeless individuals and/or families is preferred.


Experience in assessing clients' vocational and educational needs and aptitudes, along with a proven track record of successfully assisting clients in obtaining Public Assistance, Medicaid, and other benefits.

Experience in facilitating supportive group sessions.


Demonstrated sensitivity and knowledge of homelessness within marginalized communities, including individuals identifying as LGBTQ+, the elderly, and those with substance abuse or mental health challenges.

Familiarity with DHS regulations and DHS Cares Reporting.


Proven ability to deliver client services, including strong communication and listening skills, understanding of human interaction and personality development, and proficiency in case documentation and group counseling.

Proficiency in Microsoft Office Applications is required.

Flexibility in scheduling is essential.
