The primary objective is to empower clients by enhancing their skills across various life domains and preparing them for permanent housing upon completion of our program.
Key Responsibilities:
- Manage a caseload of 24 clients, ensuring personalized attention and support.
- Maintain compliance with program requirements by timely completion of individualized treatment plans, assessments, and progress notes while ensuring confidentiality.
- Conduct housing meetings for clients sharing accommodations to facilitate communication and support.
- Engage in regular contact with clients, including mandatory monthly home visits.
- Monitor clients' health conditions, including viral suppression, and make necessary referrals to healthcare services.
- Assist new residents with their transition into apartments, including setup and orientation.
- Provide referrals for additional services such as substance use support, nutrition, and mental health, prioritizing internal resources when possible.
- Ensure organized and accurate documentation of client information.
- Offer crisis intervention and mediation as required.
- Collaborate with other case managers to provide backup support when needed.
- Coordinate care across health, mental health, and substance abuse services to meet the diverse needs of clients.
- Participate in weekly supervision and team meetings, prepared to discuss casework challenges and successes.
- Utilize public transportation to accompany clients as necessary.
- Communicate effectively to advocate for clients' needs.
- Contribute to team objectives by achieving related results.
- Engage in planning and executing client activities as directed by program administrators.
- Adhere to agency policies and procedures to maintain operational standards.
Minimum Qualifications:
- Bachelor's degree in a relevant field or a Bachelor of Social Work (BSW) with a commitment to ethical social work practices, or a High School diploma with a minimum of three years of experience working with chronically homeless individuals and those with mental health challenges.
- Experience in supporting individuals affected by mental health and chronic conditions.
- Familiarity with trauma-informed care approaches.
- A minimum of three years of case management or related experience in housing program operations.
- Proficiency in electronic health records systems and Microsoft Office Suite.
- Strong organizational and time management skills.
- Excellent interpersonal skills with a collaborative team approach.
- Bilingual proficiency in English and Spanish is preferred.
