Compensation: $27.47 per hour

Role Objective:

We are seeking a knowledgeable and empathetic Case Manager to become a vital part of our organization. The successful candidate will possess experience in supporting individuals affected by mental health challenges, substance dependency, legal matters, and more. The Case Manager will be instrumental in guiding clients towards their objectives and ensuring housing security.

Position Details:

This is a full-time position that may necessitate adaptability in work hours based on the needs of the organization. The Case Manager will work collaboratively with multidisciplinary teams while also demonstrating the ability to operate independently when required.

Key Responsibilities:

  • Deliver counseling and case management services to residents
  • Collaborate with residents to formulate service plans
  • Facilitate referrals and coordinate with care agencies
  • Engage residents through home visits, outings, and appointments
  • Develop safety strategies and manage crises as necessary
  • Contribute to the creation of group activities and programs
  • Maintain confidential documentation for each resident
  • Work with other teams to promote housing stability
  • Participate in training initiatives to improve service delivery
  • Carry out additional job-related tasks as needed

Required Qualifications:

  • Bachelor's degree required; Master's degree preferred
  • Minimum of 1 year experience with at-risk populations
  • Ability to work autonomously and collaboratively
  • Familiarity with electronic case management systems
  • Experience with evidence-based practices is preferred

Preferred Qualifications:

  • Understanding of mental health, substance use, or chronic homelessness issues

Work Environment:

This role is situated within a supportive housing environment and involves working with individuals facing a variety of challenges.

Physical Requirements:

The employee may need to stand, walk, sit, and lift light objects as part of their duties.
