Supervisor: Case Management Supervisor

Compensation: $27.47 per hour

Lantern Community Services Inc is in search of a dedicated and skilled Case Manager with a background in assisting individuals who have experienced homelessness and mental health challenges within a supportive housing framework. The Case Manager will play a crucial role in helping clients stabilize their lives, maintain housing security, and achieve their personal, emotional, vocational, educational, and social aspirations. The ideal candidate will possess strong clinical expertise and experience in working with individuals facing mental health, substance use, legal, and medical challenges. A passion for helping others, along with proactive problem-solving skills, is essential for success in this role.

Key Responsibilities:

  • Deliver integrated counseling and case management services that engage clients and foster health, wellness, and personal development.
  • Collaborate with clients to create individualized, recovery-focused service plans that outline both short-term and long-term objectives.
  • Employ a harm reduction strategy to support a diverse group of formerly homeless individuals dealing with mental health and substance use issues.
  • Facilitate referrals and act as a liaison with various service providers, including health and mental health agencies, substance use programs, and employment services.
  • Engage clients through regular visits, community activities, and assistance with appointments as necessary.
  • Develop safety plans for clients and work collaboratively with the team to manage crises effectively.
  • Assist in the design and execution of programs and group activities tailored to the needs of residents, promoting an inclusive community.
  • Maintain confidential records for each client, documenting all relevant interventions and communications with external services.
  • Work alongside the Leasing and Compliance team to ensure ongoing housing stability for clients.
  • Participate in staff meetings and individual supervision to uphold best practices in service delivery.
  • Engage in a comprehensive training program focused on evidence-based practices to provide the highest quality of service.
  • Perform additional job-related duties as assigned.

Qualifications:

  • Bachelor's degree required, preferably in social work, sociology, psychology, or a related field; Master's degree preferred.
  • A minimum of one year of experience working with single adults affected by poverty, chronic homelessness, mental health issues, substance abuse, or medical challenges is essential.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Strong organizational skills and the capacity to manage a varied caseload effectively.
  • Ability to perform well under pressure, particularly in crisis situations, and flexibility in work hours when needed.
  • Demonstrated history of proactive, strengths-based engagement with individuals and families.
  • Familiarity with electronic case management systems, such as ETO, is preferred.
  • Experience with evidence-based practices, particularly Motivational Interviewing, is a plus.
  • Preferred: Knowledge of populations experiencing mental health challenges, substance use issues, or chronic homelessness.

Work Environment: This position operates within a supportive housing setting, serving individuals with mental health challenges, addiction, disabilities, HIV, and other chronic health conditions.

Physical Requirements: The role involves regular standing, walking, sitting, stooping, climbing stairs, and kneeling. Occasionally, the employee may need to lift or move items weighing up to 10 pounds.
