Position Overview: The Case Manager plays a vital role in our supportive housing initiative, specifically designed for individuals who have experienced homelessness or are living with HIV/AIDS. This position is crucial in helping clients achieve self-sufficiency and improve their quality of life.

Key Responsibilities:

  • Establish and maintain comprehensive client files and documentation.
  • Conduct initial evaluations and periodic reassessments of client needs.
  • Assist clients in overcoming obstacles to their goals and empower them to advocate for their own needs.
  • Track client progress through regular communication and in-person meetings, documenting updates accordingly.
  • Engage in collaborative case discussions with team members.
  • Facilitate bi-monthly workshops aimed at enhancing clients' independent living skills.
  • Compile and submit necessary information for regular reporting purposes.

Qualifications:

  • A Bachelor's degree coupled with two years of relevant experience.

Preferred Experience:

  • Experience working with individuals facing challenges related to HIV, mental health issues, or substance use.

Compensation: The annual salary for this position is $45,000. Salary offers are determined based on various factors, including experience, education, and program requirements.

Employment Status: This is a full-time position requiring 35 hours of work per week.
