Job Summary

Lantern Community Services is seeking a skilled and compassionate Case Manager to join our team. As a key member of our supportive housing team, you will work closely with formerly homeless, mentally ill single adults to provide integrated counseling and case management services that promote health and wellness, personal growth, and housing stability.

Key Responsibilities
  • Provide individualized counseling and case management services to residents, focusing on their unique needs and goals.
  • Develop and implement client-centered service plans that address short- and long-term objectives.
  • Collaborate with residents, healthcare providers, and community agencies to ensure comprehensive support and services.
  • Conduct regular apartment visits, community outings, and escorts to appointments as needed.
  • Develop and maintain accurate, confidential records for each resident, documenting all relevant interventions and assessments.
  • Participate in staff and clinical meetings, individual supervision, and training programs to ensure best practices and evidence-based models.
Requirements
  • Bachelor's degree in a related field, such as social work, sociology, or psychology; Master's degree preferred.
  • At least one year of experience working with single adults impacted by poverty, chronic homelessness, mental illness, substance abuse, and/or medical issues.
  • Ability to work independently and as part of an interdisciplinary team.
  • Highly organized and able to manage a diverse caseload of clients with diverse needs.
  • Strong communication and interpersonal skills, with a focus on building trust and rapport with residents.
  • Experience with electronic case management systems and evidence-based models, such as Motivational Interviewing, preferred.
Work Environment

This role operates in a supportive housing site and works with a population who may suffer from mental illness, substance abuse, disability, HIV/AIDS, and/or other health issues. The ideal candidate will be comfortable working in a fast-paced, dynamic environment and be able to adapt to changing circumstances.

Physical Demands

While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally, the employee will have to lift and/or move up to 10 pounds.
