Hudson Guild is seeking a highly skilled and compassionate Supportive Housing Specialist to join our team. As a key member of our social services team, you will provide critical support to tenants in Supportive Independent Housing, managed by Clinton Housing Development Company.
Key Responsibilities- Provide case management and housing support to tenants, ensuring they meet their goals and maintain stable housing.
- Assess client needs, linking them to relevant services and developing a plan to achieve stabilization and maintenance.
- Screen clients for public benefits and entitlements, facilitating the collection of necessary documentation.
- Engage with residents through outreach and provide information on available entitlements.
- Develop community partnerships and refer residents to relevant community providers.
- Advocate on behalf of residents to secure entitlements and maintain their stability.
- Maintain accurate and timely records of client files, progress notes, and service plans using AWARDs.
- Collaborate with colleagues and mental health professionals to ensure comprehensive support.
- Participate in tenant selection committees and attend all tenant and staff meetings.
- Produce regular reports and maintain positive relationships with Hudson Guild and Clinton Housing programs.
- Bachelor's degree in a related field, with a focus on social services.
- Bilingual in Spanish, preferred.
- Experience working with formerly homeless adults and ability to work independently.
