Reports to: Case Management Supervisor

Hourly Compensation: $27.47

Lantern Community Services Inc is in search of an enthusiastic and skilled Case Manager with a background in assisting individuals who have previously experienced homelessness and mental health challenges within a supportive housing framework. The Case Manager will play a crucial role in helping clients stabilize their lives, secure their housing, and achieve their personal, vocational, educational, and social aspirations. The ideal candidate will possess a strong clinical foundation and experience working with individuals facing mental health issues, substance use disorders, legal challenges, and medical concerns. A genuine passion for aiding others, along with the ability to think creatively and solve problems proactively, is essential for this role. The Case Manager must be capable of working both collaboratively within a multidisciplinary team and independently.

Key Responsibilities:

  • Deliver integrated counseling and case management services that effectively engage clients, fostering health, wellness, personal development, and housing stability.
  • Collaborate with clients to create personalized, recovery-focused service plans that outline both short-term and long-term objectives.
  • Employ a harm reduction strategy to support a diverse caseload of formerly homeless individuals dealing with mental health and substance use challenges.
  • Facilitate referrals and act as a liaison with various care coordination agencies, healthcare providers, mental health services, substance use programs, and employment resources to secure necessary services.
  • Engage clients through regular home visits, community activities, and accompanying them to appointments as required.
  • Develop safety plans for clients and collaborate with the team to de-escalate situations and manage crises effectively.
  • Assist in the creation and execution of group programs that cater to the needs of residents and promote an inclusive, respectful community.
  • Maintain confidential records for each client in accordance with mandated schedules, documenting all relevant interventions, assessments, and communications with external services.
  • Work closely with the Leasing and Compliance team to ensure ongoing housing stability.
  • Participate in staff meetings, clinical discussions, and individual supervision to uphold best practices in fulfilling role responsibilities.
  • Engage in a comprehensive training program that includes evidence-based models to ensure the highest quality of service delivery to clients.
  • Perform additional job-related duties as assigned.

Qualifications:

  • Bachelor's degree required, preferably in a related field such as social work, sociology, or psychology; Master's degree preferred.
  • A minimum of one year of experience working with single adults affected by poverty, chronic homelessness, mental health issues, substance abuse, and/or medical conditions is essential.
  • Ability to work autonomously as well as collaboratively within an interdisciplinary team.
  • Highly organized with the capacity to manage a diverse caseload of clients with varying needs.
  • Able to perform well under pressure, including in crisis situations, and demonstrate flexibility in work schedule when necessary.
  • Proven track record of proactive, strength-based engagement with individuals and families.
  • Experience with electronic case management systems is preferred.
  • Familiarity with evidence-based practices, particularly Motivational Interviewing, is a plus.
  • Preferred: Knowledge of populations experiencing mental health challenges, substance use issues, or chronic homelessness.

Work Environment: This position operates within a supportive housing setting, engaging with individuals facing mental health, addiction, disability, HIV, and other chronic health conditions.

Physical Requirements: While performing the duties of this role, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally, the employee may need to lift and/or move up to 10 pounds.
