A reputable nonprofit organization is seeking to enhance its Transitional Housing team. The Case Manager will play a pivotal role in delivering essential case management services to individuals facing homelessness, including those with mental health challenges and/or substance use disorders, to facilitate their journey towards stable housing.


Key Responsibilities:
  • Deliver direct support to a caseload of around 15 clients, encompassing necessary assessments, benefits navigation, and additional services aimed at helping clients achieve their housing and recovery objectives.
  • Conduct comprehensive psychosocial evaluations.
  • Link clients to psychiatric care, medical services, and substance use treatment facilities as needed.
  • Assist clients in obtaining identification, public benefits, and other relevant income sources.
  • Engage with clients weekly to evaluate their needs, promote harm reduction strategies, and formulate service plans.
  • Prepare all necessary documentation for housing applications.
  • Guide and support clients through the process of securing permanent housing.
  • Maintain accurate records in accordance with agency policies and the requirements of City and State agencies.
  • Intervene during crisis situations and accompany clients to appointments when necessary.
  • Provide support for self-administration of medication in line with organizational policies and procedures.
  • Perform additional related duties as assigned.
Qualifications:
  • Bachelor's Degree.
  • Proficient in computer usage.
  • Skilled in Microsoft Office Suite (Excel, Word, etc.).
  • Strong interpersonal abilities.
  • Exceptional communication skills, both written and verbal.
  • Meticulous attention to detail.
  • Highly organized.
Preferred Skills:
  • Over 2 years of experience working with homeless populations and addressing substance abuse issues.
  • Familiarity with Harm Reduction and Motivational Interviewing techniques.
  • Bilingual in Spanish.
