Program Overview:

Project Renewal is dedicated to providing supportive housing solutions for individuals facing various challenges, including mental health issues, substance use recovery, and chronic health conditions. Our mission is to empower residents through comprehensive support services.

Key Responsibilities:

The Case Manager plays a vital role in our team, responsible for:

  • Conducting intake assessments and developing tailored treatment plans for each resident.
  • Facilitating home visits and coordinating necessary medical, psychiatric, and substance use services.
  • Monitoring residents' adherence to prescribed medications.
  • Maintaining accurate and timely documentation of all interactions and interventions in the designated case management system.
  • Leading individual and group counseling sessions to provide emotional support.
  • Organizing educational workshops and activities that address residents' interests, such as financial literacy and job readiness.
  • Connecting residents with community resources and services.
  • Performing additional duties as assigned by supervisory personnel.

Qualifications:

  • A Bachelor's degree in social work, psychology, or a related field from an accredited institution.
