We provide over 180 integrated programs spanning various sectors including Education, Family Support, Employment Services, Health, Housing, and Legal Assistance, reaching more than 73,000 individuals and families annually.
Facility Overview: The James Bryant Homes, managed by the Addiction Rehabilitation Center Foundation, Inc., is a residential facility situated in East Harlem. This establishment offers 32 supportive housing units specifically designed for single adults (18 years and older) who are experiencing homelessness or are at risk, particularly those diagnosed with serious mental health issues or co-occurring disorders. CAMBA, Inc. serves as the primary service provider at this location, delivering case management and round-the-clock security.
Role Summary: As a Case Manager, you will play a crucial role in supporting individuals and families in need of social services.
Key Responsibilities:
- Assist clients in navigating social service resources.
- Conduct interviews and assessments to develop tailored service plans and objectives.
- Guide tenants in achieving their personal goals by connecting them with community resources and making appropriate referrals.
- Perform home visits to assess clients' living conditions.
- Support tenants in overcoming obstacles to their goals and promote self-advocacy and independence.
- Facilitate group sessions and workshops focused on independent living skills.
- Track and document tenants' progress through regular communication and visits.
- Act as an advocate for tenants with external organizations concerning education, healthcare, housing, and legal matters.
Qualifications:
- A Bachelor's degree (e.g., B.A., B.S.W.) along with two years of relevant experience or equivalent qualifications.
Compensation: The annual salary ranges from $40,000 to $45,000, determined by various factors unique to each candidate and program.
Employment Status: This is a full-time position (35 hours per week).
