Position Overview

The J.D. McCarty Center is a vibrant and innovative organization dedicated to enhancing the lives of our patients and their families. We cater to children aged 0-21 in diverse environments, including inpatient, outpatient, and educational settings.

The C.A.R.E. Program at the J.D. McCarty Center is seeking a dedicated Case Manager to collaborate with patients and their families to identify their needs and aspirations for ongoing care and development. This role involves linking patients and caregivers with suitable support services and resources available both at the J.D. McCarty Center and within the broader community.

Compensation: Starting salary of $42,500 annually, along with benefits such as Paid Time Off, Insurance, and Retirement plans. This position does not require on-call or weekend hours.

Key Responsibilities:

  • Facilitates connections between patients, families, and available support services at the J.D. McCarty Center and statewide resources.
  • Evaluates training and development needs for families and caregivers, referring them to the CARE Team Clinician for further assistance.
  • Maintains accurate documentation in accordance with agency policies, including progress notes and billing statements.
  • Coordinates outreach services with CARE team members and other staff for training and support in various settings.
  • Participates in agency, community, and state committees aimed at achieving departmental goals.
  • Implements patient care by assessing treatment needs and developing comprehensive treatment plans.
  • Supports caregiver and patient objectives through intake interviews and evaluations.
  • Establishes treatment plans by coordinating services and resources for families.
  • Monitors case status by evaluating treatment responses and advocating for necessary services.
  • Communicates client progress through interdisciplinary meetings and identifies barriers to success.
  • Prepares discharge plans by reviewing and coordinating necessary requirements.
  • Handles intake calls and manages new referrals, ensuring proper documentation.
  • Assists in maintaining quality assurance measures and prepares reports for the CARE team.
  • Processes internal referrals to various services within the center.
  • Performs additional duties as assigned.

Preferred Qualifications:

Applicants should possess a bachelor's degree in a human services field and have at least one year of experience working with individuals with intellectual and developmental disabilities. A valid Oklahoma nursing license is also acceptable with relevant experience. Preference will be given to candidates who are bilingual and have proficiency in Microsoft Office Suite and Office 365.
