Senior Care Coordinator

Become a vital part of an interdisciplinary team dedicated to assisting older adults in maintaining their independence through the role of a Springwell Case Manager.
*This position includes a SIGN-ON BONUS*
A Case Manager is responsible for performing thorough evaluations with older adults to assess their requirements and eligibility for in-home support services.
Key Responsibilities:
- Conduct in-person assessments during home visits and accurately document findings in an electronic statewide database.
- Collaborate with an interdisciplinary team to develop personalized care plans and coordinate services with a network of partner agencies.
- Engage in effective communication and consultation with family members, fellow Springwell staff, and community professionals.
Work Environment:
This role offers a hybrid work setting (combination of in-office and remote work), with no requirements for weekends, evenings, nights, or holidays. Flexible scheduling options are available.
Qualifications:
- A Bachelor’s Degree or higher is preferred.
- Applicants lacking a Bachelor’s Degree may be considered if they possess:
- An Associate's Degree coupled with a minimum of 3 years of experience in elder care or case management; OR
- Completion of at least 50% of the coursework necessary for a Bachelor’s Degree, with a commitment to finish the remaining coursework within a 3-year timeframe.
- Proficiency in computer usage, data entry, and report generation.
- Possession of a reliable vehicle, a valid driver’s license, and a clean driving record.
