We are seeking a highly motivated and compassionate Care Coordinator to join our team at Springwell. As a Care Coordinator, you will play a vital role in introducing older adults, individuals with disabilities, and their caregivers to our agency and assisting them in navigating the network of available resources.

Key Responsibilities
  • Review resources and available services, including those through the state-subsidized Home Care program.
  • Empower consumers to be in control of their decisions for long-term care.
  • Create care plans and coordinate services as directed by the consumer.
  • Work closely with consumers, caregivers, and other stakeholders to ensure seamless service delivery.
Requirements
  • Bachelor's Degree or higher in a related field.
  • Applicants without a Bachelor's Degree may be considered if they have an Associate's Degree and at least 3 years' experience working with elders or providing case management services.
  • Experience working with computers, entering data, and running reports.
  • Reliable transportation, current driver's license, and safe driving record.
