A Geriatric Care Manager is responsible for performing thorough evaluations with senior clients to assess their requirements and eligibility for in-home assistance.

Key Responsibilities:

  • Conduct in-person assessments during home visits and accurately document findings in a statewide electronic database.
  • Collaborate with an interdisciplinary team to develop tailored care plans and facilitate services through a network of partner agencies.
  • Engage in effective communication and consultation with family members, Springwell staff, and community professionals.

Work Environment:

This role offers a hybrid work setting (combination of in-office and remote work). There are no requirements for weekend, evening, night, or holiday shifts. Flexible scheduling options are available.

Qualifications:

  • A Bachelor's Degree or higher is preferred.
  • Applicants without a Bachelor's Degree may be considered if they possess an Associate's Degree along with a minimum of 3 years of experience in elder care or case management; OR have completed at least 50% of the coursework required for a Bachelor's Degree and agree to finish the remaining coursework within three years.
  • Proficiency in computer usage, data entry, and report generation is essential.
  • Applicants must have access to a reliable vehicle, a valid driver's license, and a clean driving record.
