Position Responsibilities

The Case Manager (CM) is responsible for ensuring high-quality, patient-centered care by managing rehabilitative services to achieve optimal outcomes. Key responsibilities include:

  • Providing orientation to patients, families, and caregivers regarding the case manager's role, rehabilitation philosophy, and discharge criteria.
  • Coordinating patient education and promoting active participation in care discussions.
  • Implementing individualized treatment plans that reflect the patient's strengths and preferences.
  • Documenting findings from assessments and communicating patient needs effectively.
  • Fostering positive patient interactions to enhance service quality.

Qualifications

  • Current RN licensure; BSN preferred.
  • Minimum of 5 years nursing experience, including critical care or case management.
  • Preferred certification in case management or utilization review.
  • Experience with InterQual guidelines is a plus.

Work Schedule

  • 2 shifts within a 4-week period.
