Role Responsibilities

The Case Manager (CM) is responsible for delivering high-quality, patient-focused care by overseeing rehabilitative services to achieve optimal patient outcomes. The CM collaborates with the Rehabilitation Program Director and Facility Case Management Director to ensure effective departmental operations.

Key Responsibilities Include:

  • Providing orientation to patients, families, and caregivers regarding the case manager's role, rehabilitation philosophy, and treatment processes.
  • Coordinating educational efforts for patients and families, promoting their involvement in care discussions.
  • Implementing individualized treatment plans that align with patients' strengths and preferences.
  • Documenting findings from evaluations and assessments, communicating patient needs effectively.
  • Ensuring positive patient interactions that enhance service quality.
Qualifications

Required Qualifications:

  • Current RN licensure in the state.
  • Preferred: Bachelor of Science in Nursing (BSN).
  • Minimum of 5 years nursing experience, including 2 years in critical care or 3 years in nursing with 2 years in case management.
  • Preferred: Certification in case management or utilization review.
  • Experience with InterQual guidelines is preferred.
Work Schedule
  • Per Diem colleagues are expected to work a minimum of five shifts within a four-week schedule.
  • At least one shift must be on a weekend, or the colleague may choose to work only weekends.
  • Availability for at least one major holiday and two minor holidays is required.
