Role Responsibilities

The RN Case Manager plays a crucial role in delivering high-quality, patient-focused care by managing rehabilitative services to achieve optimal outcomes. Responsibilities include:

  • Providing orientation to patients and families regarding the case management process, rehabilitation philosophy, and insurance benefits.
  • Coordinating educational efforts for patients and families, ensuring their active participation in care discussions.
  • Implementing individualized treatment plans that align with patient strengths and preferences.
  • Documenting assessments and communicating patient needs effectively.
  • Promoting positive patient interactions and ensuring exceptional service delivery.

Qualifications Required

  • Current RN licensure in the state; BSN preferred.
  • A minimum of 5 years nursing experience, including critical care or case management.
  • Preferred certifications in case management or utilization review.
  • Experience with InterQual is a plus.

Work Schedule

  • Per Diem colleagues are expected to work a minimum of five shifts within a four-week period, including at least one weekend shift.
  • Availability for major and minor holidays is also required.
