Role Responsibilities

The Case Manager (CM) is responsible for delivering high-quality, patient-focused care by overseeing rehabilitative services to achieve optimal patient outcomes. The CM will:

  • Conduct program orientations for patients and their families, explaining the case manager's role, rehabilitation philosophy, and discharge criteria.
  • Facilitate patient and family education, encouraging their involvement in care discussions and treatment planning.
  • Ensure the execution of personalized treatment plans that align with the patient's strengths and preferences.
  • Document findings from discharge planning evaluations and psychosocial assessments, communicating patient needs effectively.
  • Promote positive patient interactions to enhance the overall service experience.

Qualifications Required

  • Current RN licensure in the state. BSN preferred.
  • A minimum of 5 years of nursing experience, including 2 years in critical care or 3 years with 2 years in case management.
  • Preferred certifications in case management or nursing.
  • Experience with InterQual is a plus.

Work Schedule

  • 2 shifts per 4-week period.
