Key Responsibilities

The RN Case Manager is responsible for delivering high-quality, patient-focused care by managing rehabilitative services to achieve optimal patient outcomes. Key responsibilities include:

  • Conducting patient and family orientation regarding the case manager's role, rehabilitation philosophy, and discharge criteria.
  • Coordinating educational efforts for patients and families, encouraging their involvement in care discussions.
  • Implementing individualized treatment plans that align with patient strengths and preferences.
  • Documenting discharge planning evaluations and communicating patient needs effectively.
  • Promoting positive patient interactions and ensuring exceptional service delivery.
Qualifications

To be considered for this position, candidates should possess:

  • Current RN licensure; BSN preferred.
  • A minimum of 5 years of nursing experience, including 2 years in critical care or 3 years in nursing with 2 years in case management.
  • Preferred certification in case management or utilization review.
  • Experience with InterQual guidelines is a plus.
Work Schedule

As a Per Diem employee, you will be expected to:

  • Be available for at least five shifts within a four-week schedule.
  • Work at least one weekend shift or commit to working a minimum of four weekend shifts.
  • Be available for at least one major holiday and two minor holidays.
