Job Summary and Qualifications
The Case Manager (CM) is responsible for ensuring high-quality, patient-centered care by managing rehabilitative services to achieve optimal outcomes. The CM will:
  • Provide orientation to patients, families, and caregivers regarding the case manager's role, rehabilitation philosophy, and discharge criteria.
  • Coordinate patient education and promote active participation in care discussions.
  • Implement treatment plans that align with the patient's strengths and preferences.
  • Document findings from evaluations and communicate the needs of patients and families.
  • Foster positive patient interactions that enhance the quality of service.

Qualifications
To be considered for this role, candidates should possess:
  • Current RN licensure.
  • Preferred BSN degree.
  • A minimum of 5 years of nursing experience, including 2 years in critical care or 3 years with 2 years in case management.
  • Preferred certification in case management or utilization review.
  • Experience with InterQual is preferred.

Schedule
Per Diem colleagues are expected to be available for a minimum of five shifts within a four-week schedule, including at least one weekend shift and one major holiday.
