Position Summary
The Inpatient Rehabilitation Nurse Case Manager plays a crucial role in delivering high-quality, patient-focused care by overseeing rehabilitative services to achieve optimal patient outcomes. This role involves collaboration with the Rehabilitation Program Director and the Facility Case Management Director to ensure effective departmental operations.
Key Responsibilities
- Conduct program orientations for patients and their families, explaining the role of the case manager, rehabilitation philosophy, and discharge criteria.
- Facilitate patient and family education, encouraging active participation in discussions regarding care plans and goals.
- Ensure the implementation of personalized treatment plans that reflect the patient's strengths and preferences.
- Document findings from discharge planning evaluations and psychosocial assessments, communicating patient needs effectively.
- Promote positive patient interactions that enhance the overall service experience.
Qualifications
- Current RN licensure in the state; BSN preferred.
- A minimum of 5 years of nursing experience, including 2 years in critical care or 3 years with 2 years in case management.
- Preferred certifications in case management or utilization review.
- Experience with InterQual guidelines is a plus.
Work Schedule
- 2 shifts per 4-week period.
