Role Responsibilities
The Case Manager is responsible for ensuring high-quality, patient-centered care by managing rehabilitative services tailored to each patient's needs. Key responsibilities include:
- Providing orientation to patients and their families regarding the rehabilitation process, including the role of the case manager and insurance benefits.
- Coordinating educational efforts for patients and families, encouraging their involvement in care discussions.
- Implementing treatment plans that reflect the patient's strengths and preferences.
- Documenting assessments and communicating the needs of patients and families effectively.
- Promoting positive patient interactions to enhance service quality.
Qualifications Required
- Current RN licensure with a preference for BSN degree.
- A minimum of 5 years nursing experience, including critical care or case management.
- Preferred certification in case management or utilization review.
- Experience with InterQual guidelines is advantageous.
Work Schedule
- 2 shifts within a 4-week period.
