Role Responsibilities
The RN Case Manager is responsible for delivering high-quality, patient-focused care by overseeing rehabilitative services to achieve optimal outcomes. This role involves collaboration with the Rehabilitation Program Director and Facility Case Management Director to ensure effective departmental operations.
Key Responsibilities Include:
- Conducting orientation sessions for patients and their families, explaining the case manager's role, rehabilitation philosophy, and discharge criteria.
- Coordinating educational efforts for patients and families, encouraging their involvement in care discussions.
- Implementing treatment plans that align with the patient's strengths and preferences, ensuring active patient participation throughout the rehabilitation process.
- Documenting discharge planning evaluations and psychosocial assessments, communicating patient needs and preferences effectively.
- Promoting positive patient interactions that enhance service quality.
Qualifications Required:
- Current RN licensure in the state, with a preference for BSN holders.
- A minimum of 5 years nursing experience, including 2 years in critical care or 3 years with 2 years in case management.
- Preferred certification in case management or nursing.
- Experience with InterQual is advantageous.
Work Schedule:
- Per Diem staff are expected to work a minimum of five shifts within a four-week period, including at least one weekend shift.
- Availability for at least one major holiday and two minor holidays is required.
