Position Summary

The Registered Nurse Case Manager is responsible for ensuring high-quality, patient-centered care by managing rehabilitative services to achieve optimal outcomes. This role involves collaboration with the Rehabilitation Program Director and Facility Case Management Director to coordinate care tailored to each patient's needs.

Key Responsibilities
  • Provide orientation to patients and their families regarding the case manager's role, rehabilitation philosophy, and discharge criteria.
  • Facilitate patient and family education, promoting active participation in care discussions.
  • Implement treatment plans that align with the patient's strengths and preferences.
  • Document findings from assessments and communicate any social or financial needs of the patient.
  • Ensure positive patient interactions that enhance the quality of service.
Qualifications

To be considered for this role, candidates should possess:
  • Current RN licensure.
  • Preferred BSN degree.
  • A minimum of 5 years of nursing experience, including critical care or case management experience.
  • Certification in case management or related fields is preferred.
  • Experience with InterQual is a plus.
Work Schedule

This is a per diem position, requiring availability for a minimum of five shifts within a four-week schedule, including weekends and holidays.
