RN Case Manager PRN

The RN Case Manager PRN is a key member of the healthcare team at HCA Florida Aventura Hospital, responsible for ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimal outcomes.
Key Responsibilities- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- RN with current state licensure; BSN Bachelor's prepared preferred.
- 5 years of nursing experience with 2 years critical care or 3 years nursing experience with 2 years case management experience preferred.
- Certification in case management, nursing, or utilization review preferred.
- InterQual experience, preferred.
Per Diem Colleagues agree to be available to work a minimum of five (5) shifts within each four-week schedule, including one (1) Weekend shift or four (4) weekend shifts within each four-week Schedule, and at least one (1) shift on one (1) major holiday and (2) minor holidays.
