The Rehabilitation Nurse Case Manager plays a vital role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes. This position requires a skilled and compassionate nurse to provide, coordinate, and direct care specific to the needs of each rehabilitation patient.
Key Responsibilities- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- Currently licensed as an (RN) Registered Nurse in the state(s) of practice according to law and regulation.
- Associates degree required, BSN preferred.
- 2+ years experience in case management or 3+ years experience in clinical nursing (i.e. acute care area: ICU, PCU, Med Surg- Tele, Oncology) Required.
- InterQual experience Preferred.
- Certification in Case Management preferred.
HCA Florida Mercy Hospital is a 488-bed facility, accredited by the Joint Commission, offering a wide range of medical specialties, including emergency care, heart, and vascular care, and maternity and neonatal care. We are a Level II NICU hospital and CARF accredited for inpatient rehabilitation. Our facility offers a Comprehensive Community Cancer Program, accredited by the American College of Surgeons Commission on Cancer. We are affiliated with HCA Florida Healthcare Division and sponsored by the Sisters of St. Joseph of St. Augustine, Florida.
