Key Responsibilities

The RN Case Manager plays a vital role in ensuring high-quality, patient-focused care by managing rehabilitative services to achieve optimal patient outcomes. Responsibilities include:

  • Providing orientation to patients and their families regarding the case management process, rehabilitation philosophy, and discharge criteria.
  • Coordinating education for patients and families, encouraging their involvement in care discussions.
  • Implementing individualized treatment plans that reflect the patient's strengths and preferences.
  • Documenting assessments and communicating patient needs effectively.
  • Promoting positive patient interactions and maintaining high standards of service.
Qualifications
  • Current RN licensure in the state; BSN preferred.
  • Minimum of 5 years nursing experience, including critical care or case management.
  • Preferred certifications in case management or utilization review.
  • Experience with InterQual criteria is advantageous.
Work Schedule
  • Per Diem colleagues are expected to work a minimum of five shifts in a four-week period, including weekends and holidays.
