Job Summary and Qualifications
The Case Coordinator (CC) guarantees high-quality, patient-centered care by overseeing rehabilitative services to ensure optimal outcomes. The CC provides, coordinates, and directs care tailored to the unique needs of each rehabilitation patient. The CC collaborates with the Rehabilitation Program Director and Facility Case Management Director regarding departmental operations. The CC coordinates efforts within the Rehabilitation team.

Responsibilities in this role:
- You will provide program orientation to patients, families, and caregivers. This orientation will encompass the case coordinator's role, rehabilitation philosophy, and criteria for continued stay and discharge. It will also include information on Medicare and insurance benefits, grievance procedures, treatment plan processes, and rights and responsibilities.
- You will serve as the coordinator for patient, family, and caregiver education, promoting their involvement in team discussions regarding plans, goals, and status through Family Conferences and other interactions.
- You will ensure the execution of the patient's treatment plan that aligns with their strengths, abilities, needs, and preferences, facilitating their involvement throughout the rehabilitation process.
- You will document findings from the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating the social, financial, or discharge needs and preferences of the patient, family, or caregiver.
- You will take responsibility for fostering consistent, positive patient interactions that advance the agenda of exceptional patient service.

Qualifications:
- RN with current state licensure. BSN preferred.
- 5 years of nursing experience with 2 years in acute care (e.g., ICU, Med-Surg, Tele, PCU) or 3 years of nursing experience with 2 years in case management preferred.
- Certification in case management, nursing, or utilization review preferred.
- InterQual experience preferred.
