Work Location : UHealth TowerThe University of Miami/UHealth has an exciting opportunity for an RN Case Manager in the Case Management department at the UTower location.
This position functions as a RN Case Manager for the Case Management Department under the direction of the Director of Case Management.
- Incorporates the fundamental principles of utilization management, monitoring resource consumption, social services intervention and individual discharge planning that will include assuring that the transfer or discharge of a patient to another level of care, treatment, services or different setting is always based on the patient's assessed needs and the organizations capabilities to meet these needs.
- Facilitates delivery of clinical and community services to patients and families through effective utilization of available resources.
- Manages the observation patients on their assigned unit, conduct InterQual reviews for admission, and continued stay per policy, report avoidable days and facilitate clinical reviews to third party payors
- Ensures the appropriateness and cost effectiveness of patient's plan of care. Report all complex cases to the Director and/or Care Coordination Manager as often as necessary.
- Proactively collaborates with physicians(s) to develop patient care plans and review for medical necessity for continued hospital services and resource consumption.
MINIMUM QUALIFICATIONS:
- BSN graduate of an accredited school of nursing.
- Valid State of Florida RN license.
- Minimum of one (1) year in Hospital Case Management/nursing.
- Working knowledge of patient assessment, medical terminology.
- Current BLS - CPR Certification.
