Job Overview

As a vital member of our team, the Case Manager plays a crucial role in delivering exceptional care and support to individuals and families in need. This position requires collaboration with a diverse group of social service professionals to ensure compliance with established guidelines while promoting the mission of St. Joseph Center.

Key Responsibilities

The Case Manager is responsible for:

  • Conducting outreach and educational initiatives aimed at enhancing mental health awareness among Latino communities.
  • Executing routine case management tasks in partnership with program staff to deliver comprehensive services to clients.
Essential Duties

The essential duties include but are not limited to:

  • Assisting clients in accessing necessary community resources and support systems.
  • Coordinating service provision as per agency guidelines and funding requirements.
  • Working collaboratively with staff to organize activities, recruit participants, and manage event logistics.
  • Facilitating therapeutic group sessions and contributing to the development of relevant curricula.
  • Providing individual rehabilitation support by teaching coping strategies to enhance mental wellness.
  • Participating in regular team meetings focused on Latino community services.
  • Maintaining accurate documentation of all advocacy and case management efforts as mandated by the agency and funding sources.
Qualifications

Knowledge, Skills & Abilities:

  • Fluency in both Spanish and English is required.
  • A strong interest in immigration issues and family dynamics is advantageous.
  • Self-motivated with excellent communication skills to engage with a diverse population.
  • Strong organizational and administrative capabilities.

Experience:

  • Prior experience in mental health and substance use case management within community settings, particularly with at-risk populations, is preferred.

Education:

  • A Bachelor’s degree in social work, human services, or a related field from an accredited institution.

Additional Requirements:

  • Proficiency in MS Office Suite and familiarity with internet browsers.
  • A valid California Class C Driver License and automobile liability insurance are necessary.
  • Willingness to travel for client meetings and transport clients as needed.
Work Environment

The work environment is typically moderate in noise level, and the physical demands include standing, sitting, walking, and occasional lifting of up to 20 lbs. Reasonable accommodations may be made for individuals with disabilities.
