Job Summary

The Case Manager I - Multifaceted Intervention Specialist serves as a vital team member with social service professionals and paraprofessionals to deliver high-quality care in accordance with the rules and regulations established by the Los Angeles Department of Mental Health for contracted agencies while adhering to the mission of St. Joseph Center.

Key Responsibilities
  • Outreach and Engagement
    • Provide targeted outreach, engagement, and education activities to Latino individuals and families to increase awareness regarding overall mental health and health issues.
  • Case Management
    • Perform routine case management activities while working with other program staff to provide services to clients.
  • Community Resource Coordination
    • Help prospective clients gain access to needed community resources and support.
  • Service Coordination
    • Assist in establishing services as defined by agency and funding sources.
  • Program Development and Implementation
    • In conjunction with other staff, coordinate activities, recruit participants, arrange event logistics, and provide staff support.
  • Therapeutic Group Facilitation
    • Facilitate therapeutic groups, including working alongside other staff to develop and implement curriculums.
  • Individual Support
    • Assist and explore individual rehab sessions by providing coping skills to help support clients with mental well-being.
  • Team Collaboration
    • Participate in regular Latino ISM team meetings.
  • Documentation
    • Document all advocacy and case management activities as required by agency and/or funders.
Requirements
  • Language Proficiency
    • Fluency in Spanish/English language required.
  • Knowledge and Skills
    • Interest in immigration issues and/or family systems is a plus.
    • Must be highly motivated and a self-starter.
    • Must have the ability to communicate with and relate to a diverse group of people including clients, community, and other staff.
    • Must have excellent administrative and organizational skills.
  • Experience
    • Experience providing mental health and substance use case management services in a community setting with at-risk populations, experience in Hispanic culture is preferred.
  • Education
    • Bachelor's degree from an accredited college or university in social work, human services, or a related field.
  • Computer Skills
    • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and widely supported internet browsers.
  • Certifications and Licenses
    • Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
    • Valid automobile liability insurance.
  • Travel Requirements
    • Travel to meet clients may be required.
    • Transport clients using company vehicle as needed.
